Patient declarations for Coach

If I book an appointment for nutritional coaching via The Body Clinic platform, I declare to the coach I have chosen:

• I have taken note of the coaches’ privacy policy.
• I consent to the processing of my personal health data by my chosen coach for the purpose of conducting the coaching session. I can revoke my consent to the coach at any time with effect for the future.
